How difficult it may be to swap a manual transmission into a 20B Eunos Cosmos.

It has been done multiple times before. IIRC, it’s recommended to used the transmission from the 2nd-gen (FC) RX-7 turbo, as it’s Mazda’s strongest compatible one (the FD box has synchro issues). For the pedal box and slave cylinder, I think that 626 parts are compatible? IIRC, you can use FC parts as well, but FD parts require more modification. Reinforcement of the sheet metal above the pedal box is also recommended, as it was never designed to take the forces of clutch actuation.

It’s not super easy... But it's been done multiple times before so it's not like it'll be super expensive or difficult. It'll be more work than a normal manual swap though.

It’s certainly do-able, as others have said, but you’d need to be sure you wanted to. I’ve got a friend who owned and loved an auto one, then stupidly got a bee in his bonnet about fuel costs and sold it. A few years later he had sellers regret and bought another one, this one with a manual conversion. He only kept it about 3 months - he hated the MT, said it totally ruined the character of the car and the engine. These things are Mazda’s equivalent of an S Class Coupe or an 8 Series: they’re big GTs not sports cars. MT isn’t necessarily the right answer.

I believe production started in April of 1990 so you can find a few that can be imported under the 25 year law already.

The problem is that they only made, as far as I can tell, a few thousand 20B models. Which make me wonder if the ones for sale here have been sitting and piling up because no one in Japan wants one, or are exorbitantly overpriced over there.

Yeah, no one wants them here and they are way too expensive in North America :)
